This page lets you change your Benefit Amounts. All Medical, Dental, Vision and Hearing wellness Medical Services are included.

To change your Medical Services Benefit Amounts and to add Prescription Medications, click the field and select your Benefit Amount from the drop-down list. For help and guidance, click the icon "i" for more helpful information or go to FAQs or contact your CHC Representative.

When completed, click the "Apply" button or "Next" to go to the next Customize page. Only a few steps remain.

This page lets you add two Coverages and Benefits that lower your Out-of-Pocket cost and financial risks.

The CHC Loss of Income Subsidy pays your CHC Healthcare Plan monthly price if and when you experience a loss of income due to a Medical Event. Lose your job and income, you won’t lose your CHC Healthcare plan.

The ACA & Group Financial Subsidy will pay your ACA or Group Insurance plan’s monthly premium if you experience a major medical issue and need additional medical cost protection.

To add either of these Coverages, select the number of months you want the Coverage. Options are 1 to 12 months. You’ll see the Benefit Amount and the monthly price. For the ACA & Group Financial Subsidy you also need to select which person has the Coverage. Options are You, your Spouse/Partner or Both of You. The CHC Loss of Income Subsidy only applies to you, the Owner of the policy.

Your CHSA is similar to a federal CHSA with fewer restrictions and greater benefits. Your CHSA increases your Medical Services Benefit amounts.

You can use your CHSA to pay for any Medical, Dental, Vision, Hearing medical service or approved Medical Product. CHSA increases your overall Plan Benefit Amounts.

You select the amount you want deposited each month to your CHSA. CHC matches "dollar-for-dollar" your CHSA deposit Plus, CHC pays 5% interest on your CHSA deposits. Better than a bank savings account! Your selected CHSA deposit amount will be included in your CHC Healthcare Plan monthly price.
