Welcome:  Here's a few things you need to know.  We respect your privacy.  We won't ask you for your name or contact information to give you a quote.  If you want our help, you'll need to ask us. 

Getting a Quote:  If you prefer, we'll do the Quote for and with you.  Otherwise, you can Design your own Company Healthcare Plan using this page.  It'll take about 10 minutes to complete.  Longer if you want to learn as you go.

Directions:  Click the Step 1 Blue Bar and that section will open.  Answer a few questions and move to the next Step.  As you enter your information and selections, you will see your Company Plan Price change.  Each page has some directions and tips. More detailed information can be found in the FAQ section.

Plan Design Steps:  Steps 1 & 2 collects information on who you want included in your Quote.  You can include anyone; Full-Time, Part-Time, 1099s, even volunteers.  Collectively, we call then your "Staff".
Steps 3, 4, & 5 lets you choose the Coverages and Benefits you want to include in your CHC Healthcare Plan.  Step 6 shows a comparison between your CHC Healthcare Plan and a typical "Group" insurance plan.  Step 7 is you contact information.

When Completed:  You can send your Design to us for review.  We might have some suggestions.  Click the "Send Us Your Plan" HUG.

Description:  This Step collects.
Ager Tier:  The Age Tier collects the "Number" of Staff members you want included in your Quote.  Enter the number of Staff in each appropriate Age Tier age range.  Example, if you have 10 Staff members with ages between 20 & 29, enter 10 in that Age Tier row.
Optional Age:
  The Quote form uses averages unless you enter a specific age.  You can enter an exact age in the Optional Age field.  Example, if you have 3 Staff members age 23, 35, 42, enter those ages in the appropriate Age Tier row.

Retail Price, Discounts, CHSA:  These calculations can be selected by you in the other Blue Bars.

Description:  This Step tells us about the makeup of your Company's Staff.   The Average Staff Income, Average Age, Average Health Class compute CHC Discounts and Subsidies.

Description:  Unlike a traditional "Group" healthcare plan, CHC lets you design your healthcare plan.  Select the Coverages and Benefits you want to offer.  Your Staff can further personalize their plan to meet their needs and budget.

Description:  The Medical Services Benefit is the amount allocated to pay a Staff member's Medical Services.  The CHC and ACA/ Group Plan Subsidy is the Plan Benefit Amount that will pay the Staff member's CHC, ACA or Group plans monthly cost.  Enter the number of months.  The Benefit Amount is calculated.

Description:  Similar to a federal HSA, CHC will establish for each Staff member a CHSA account.  Enter the desired monthly amount in the CHSA Amount per Affiliate field.  The rest of the fields are calculated for you.

Future CHSA Balance:  Enter a month in the Month field and see the CHSA Account Balances.

Description:  This Table shows a comparison between the CHC Healthcare Plan you designed and a typical ACA or Group insurance plan.  Out-of-Pocket is the CHC Plan's Co-Pay and the ACA Plan's Deductible plus Co-Insurance costs.  Your CHC has no Deductible or Co-Insurance costs.  All-in-Cost is the total of the Annual plus Out-of-Pocket cost.
